Clients are responsible for:
- Providing accurate and complete medical history and other relevant information before receiving any treatment. This includes signing all the required medical forms given by Noble Wellness and Aesthetics.
- Following all pre- and post-treatment instructions provided by Noble Wellness & Aesthetics.
- Arriving on time for appointments. Late arrivals may result in a shortened session or rescheduling.
Failure to comply with these responsibilities may affect the outcome of your treatment and could result in the cancellation of your appointment.
We accept debit and credit card payments. Payments are processed through Stripe (Dr. Chrono), Zelle, and Venmo. Full payment is required at the time of booking or at the time of service, depending on the service being provided.
Cancellation Policy: Clients must cancel or reschedule appointments at least 24 hours in advance. Failure to do so will result in a minimum of $20 cancellation fee. Prices are subjected to increase based on treatment.
Refund Policy: All sales are final. No refunds will be issued for services rendered, including individual services or packaged deals. If a service package is purchased, all services within the package must be used within the specified time frame. No refunds or credits will be provided for unused services within a package.
At Noble Wellness & Aesthetics, we are committed to protecting your personal health information in accordance with the Health Insurance Portability and Accountability Act (HIPAA) regulations. We do this by using our medical software Dr. Chrono. With this software, our medical spa ensures all sensitive client data is handled with the utmost confidentiality. This includes: - Confidentiality: Your PHI is only accessible to authorized personnel who need it to provide care or process transactions.
- Security: We implement physical, administrative, and technical safeguards to protect your PHI from unauthorized access or breaches. - Access and Disclosure: We will not disclose your PHI without your consent, except as required by law or for treatment purposes. By adhering to these practices, we strive to create a safe and secure environment where you can feel confident and radiant in your unique beauty journey. We also collect and maintain medical history, consent, and intake forms. This information is kept confidential and is only used for the purposes of providing you with services and managing your treatment plans. However, we do not sell or share your personal information with third parties, except as required by law.
By choosing Noble Wellness & Aesthetics, you acknowledge that you have been informed of the risks associated with the treatments. This means Noble Wellness is not liable for any adverse effects, injuries, or complications resulting from treatments. Clients must agree to release Noble Wellness & Aesthetics from any claims of liability, EXCEPT in cases of gross negligence. This will also be part of the forms clients must sign before receiving treatment from Noble Wellness & Aesthetics.
